Home & Community-Based Services (HCBS – 1915(i) & Innovations Waiver)
LifeBloom’s Home and Community-Based Services support individuals with intellectual/developmental disabilities and other qualifying needs in living independently and participating fully in their communities. These services focus on skill-building, daily living supports, community integration, caregiver support, and non-medical assistance delivered in home and community settings. All services are person-centered and provided in accordance with NC Medicaid State Plan and waiver requirements.

Care Management & Coordination (Tailored Care Management)
Care Management and Coordination services help individuals navigate complex systems of care by ensuring their medical, behavioral, and social needs are addressed in a coordinated manner. LifeBloom’s care managers conduct assessments, develop person-centered care plans, connect individuals to services, monitor progress, and provide advocacy during transitions or crises. Services are delivered in alignment with NC Medicaid Tailored Care Management requirements.

Adult Day Services
Adult Day Services provide structured, daytime support in a supervised, community-based setting for adults who need assistance or social engagement during the day. Services focus on life skills development, social interaction, wellness monitoring, and meaningful activities while offering relief to caregivers. Adult Day Services promote independence, routine, and quality of life without overnight care.

Housing Stabilization & Transition Services
LifeBloom’s Housing Stabilization and Transition Services assist individuals who are homeless, at risk of homelessness, or transitioning from institutional or unstable settings. Services include housing assessments, transition coordination, landlord engagement, participant goods and services, and post-transition follow-up. These supports are non-rental assistance services intended to help individuals secure and maintain safe, permanent housing.

Alternative Family Living (AFL) / Host Home Support
Alternative Family Living is a residential support program that provides individuals with a stable, family-like home environment where they receive 24/7 supervision and assistance. Participants live in a private home with a trained AFL provider who supports daily living activities, community involvement, behavioral needs, and personal growth. AFL services are authorized through NC Medicaid waivers and are designed to promote safety, independence, and long-term stability.

Home Health Care Services
LifeBloom’s Home Health Care Services deliver medical and non-medical care to individuals in their homes who need support due to illness, disability, or aging. Services may include skilled nursing, home health aide support, personal care, medication assistance, and chronic condition management. Services may be covered by Medicaid, Medicare, private insurance, or private pay, depending on eligibility.
