Sub-Services

LifeBloom Support & Referral Services, LLC provides community-based support services authorized through NC Medicaid, Tailored Plans, and LME-MCO oversight. All staff, contractors, and providers must understand the scope, purpose, and documentation expectations for each service delivered. Staff may only provide services that are approved under the agency’s enrollment, credentialing, and authorization requirements.

The following service definitions apply to LifeBloom operations and must be followed at all times:

Nutritional Services

Supports individuals with meal planning, nutrition education, and assistance to promote healthy eating and wellness.

Training and Education Services

Delivers instruction and education focused on life skills, self-advocacy, health, safety, and personal development.

Community Integration Services (CIS)

Assists individuals in accessing community activities, building social connections, and increasing independence outside the home.

Community Living & Integration Support (CLIS)

Supports individuals in developing daily living skills, independence, and meaningful participation in their home and community environments.

Housing Needs Assessment

Evaluates an individual’s housing situation, risks, preferences, and supports needed to achieve stable, safe housing.

Post-Transition Follow-Up

Provides ongoing monitoring and support after housing placement to ensure stability, safety, and long-term success.

Caregiver Training

Educates and supports caregivers with skills, strategies, and resources to safely and effectively care for their loved ones.

Participant Goods and Services

Provides essential items and supports necessary to maintain health, safety, and successful community living.

Community Transition Funding (CTF)

Offers one-time financial assistance to help individuals establish a safe and stable home during community transition

Supported Employment

Helps individuals prepare for, obtain, and maintain competitive employment through skill development and ongoing job support.

Lease Support & Move-In Assistance

Assists individuals with lease understanding, application support, deposits, and move-in coordination.

Non-Medical Transportation Services

Provides transportation to medical appointments, employment, community activities, and essential services.

Transition Coordination (MFP)

Provides coordination and planning to support individuals transitioning from institutional or congregate settings into the community.

Individual Living Supports (ILS)

Provides one-on-one assistance to help individuals live independently, manage daily tasks, and achieve personal goals.

Respite Services

Offers temporary relief to caregivers by providing short-term care and supervision in a safe and supportive setting.

Landlord Coordination

Works directly with landlords to support tenancy, address concerns, and promote successful housing placements.
